CHRC - A Community Health Resource Center Initiative

Bridging the Gap Transportation

Greensburg, St. Helena & the Florida Parishes, LA

Our initiative:

CHRC - Bridging the Gap Transportation is excited to begin providing curb-to-curb transportation service for our seniors, veterans and the disabled, or anyone needing Non-emergency Medical Transportation (NEMT).  Additionally, we will also offer a goods delivery service.  We serve the city of Greensburg, LA, St. Helena, and the Florida parishes.

We believe we have developed a streamlined and efficient service program designed to meet your needs.  We're offering this service as a bridge to fill in the gap of inadequate transportation and resource availability.   By scheduling a ride using our easily accessible online trip planner or if more convenient, you may also make arrangements for pick-up, by phone @713-705-6300.  Bear in mind that there is a 48hr advance NEMT request notice requirement.

Our on-demand goods delivery service is available for your convenience, just one click or phone call away.  However, there is a 24 Hr. advance notice requirement for this service.

INTEGRITY:   We will offer services, in a timely and efficient manner.  Scheduling begins from 6a early morning through 6p early evening, Mon - Fri.

SAFETY:   Your safety is of our utmost concern and

we will operate within the established guidelines of LaDOTD.

SUPPORT:   We will support our senior community by providing transport service  from one specific location to another. We require advanced notification of intent to ride, 24 Hrs. before your requested pickup time. This type of service is based upon demand and allows the most flexibility.

Transportation for seniors, veterans, or the disabled, is crucial to ensure access to essential services for such appointments as: Non-emergency medical, shopping, pharmacy, meal sites, social activities etc. The availability of adequate transportation enables our seniors to live independently within their community, helps to prevent isolation and the possible need for long-term care placement.

Do you have a grocery order waiting for pickup?  Worry no more; use our goods delivery service, to schedule your pickup, with one call or click away.

We have a designed an efficient transportation program to meet your needs.  We look forward to serving our greater Houston area residents. Schedule your transport service today!
